By using its robust Individual Health Profile application, BlenderHealth creates a continually changing profile of the patient. The profile is accessed through a Dashboard, and includes current patient and provider goals, any appropriate data and information about the patient. The profile provides the parameters to feed continuing health prevention and wellness plans. A Digital Portfolio allows a patient to collect and share evidence of individual success.
To understand the objectives and goals of a patient’s continuing health prevention, wellness and care plan, the Content Management System (CMS) can help the doctor, patient, and any caregivers manage ALL the relevant content that has been organized and tagged in the CMS. This allows for the appropriate linkage between the patient, doctor and caregivers to the content.
